Our staff is here to help you through the entire process. Whether you need help with paperwork or have questions about the insurance process, we’re here to help each step of the way.
We’ll Walk You Through the Process
To get started, simply call or visit your local center to speak with our staff and schedule a tour. A knowledgeable Case Coordinator will work with you to complete eligibility paperwork and schedule an assessment to help us understand the individual’s needs. We then create an individualized Plan of Care outlining the participant’s needs and which services will benefit him or her.
Revel Adult Day Health participants must have a comprehensive medical exam from their physician before joining our centers. Your Case Coordinator can work with the doctor’s office to get exam results and any other health information needed to begin the process.
Revel Adult Day Health accepts various forms of insurance, including Medicaid. We will also work with the funding source to make sure all needed screenings and paperwork are completed.
